Prior to your first visit
Find out details about making your first appointment by calling 970-245-6911 and speaking with one of our courteous and well-informed receptionists. They will assist you in determining which services at the IMC best suit your needs.
To save time on your first visit, you will be asked to complete a series of questionnaires and sign some paperwork which will be mailed to you ahead of time. Please take the time to fill out the forms completely prior to coming to the office to avoid delaying your appointment.
To make the most of your first appointment, please bring:

all available and relevant medical records

the names and addresses of all physicians you currently see (in case we need to obtain records from their offices)

a complete list of the nutritional supplements you take, with exact ingredients (it is often easier to just bring the supplements themselves).
Your initial visit may take an hour or more so please be sure to schedule yourself plenty of time. We do not want you to feel rushed at any time during your visit. Our aim is to make your first visit a comprehensive and pleasant experience.
